The Clinical Nurse Manager - Connected Care Program will oversee and lead the day-to-day efforts of our individual nurse case and care managers working in our Case, Care and Disease Management Program (Connected Care Program). The manager will be key in helping their team members achieve specific individual and team-level targets, in order to drive high quality outcomes for our members. The Clinical Nurse Manager will be a central point of contact and key collaborator as we establish and implement new operational workflows, new services and new expectations throughout our organization. As we scale, the Clinical Nurse Manager will be vital to facilitate professional growth and development among their direct team members.

Responsibilities:

  • ​​Track and actively manage the performance of all individual Nurse Care Managers under your leadership to ensure delivery of timely and high quality care.
  • Collaborate with the workforce management team to develop and maintain team and individual working schedules
  • Collaborate with the Clinical Excellence (CE) team to promote the highest level of care and service excellence possible.
  • Surface feedback and partner with the Continuing Education team on opportunities to improve service and care excellence.
  • Work together with the Continuing Education and Human Resources Business Partners to develop formal remediation plans for individuals who may struggle to meet our high standards
  • Support growth of individual team members.
  • Bridge between subject matter experts/service line managers and individual team members to implement workflow changes
  • Establish expectations for metrics for individual team members. Metrics may include goals for volumes, customer commitments, service line agreements and performance guarantees.
  • Assist with day-to-day work and direct patient care, as needed
  • Potential to attend biannual management offsites (dates/locations to be determined)

Qualifications:

  • Be a Registered Nurse in good standing with the nursing board of his/her/their state
  • Maintain a compact state license and be willing to get licensed in non compact states
  • Bachelor’s Degree 
  • 5+ years nursing experience required (Acute case management and/or longitudinal care management experience preferred)
  • 2+ years of people management experience (Care management leadership experience a plus)
  • Experience leading remote teams highly preferred
  • Excellent organizational and team-building skills, attention to detail
  • Proven experience in leading teams/initiatives with demonstrated results
  • Technology proficiency required
  • Data-driven and focused
  • Proven ability to manage change and learning agility
