Responsibilities include conducting Spanish-language sessions, creating individualized treatment plans, documenting care in an EMR with AI assistance, collaborating with the care team, and participating in case consultations and clinical leadership meetings.

Provide comprehensive telehealth assessments for older adults with chronic and complex conditions. Identify clinical risks, care gaps, medication concerns, and recent utilization issues. Oversee individualized care plans with healthcare advocates, address medical and social needs, and collaborate with therapists and care teams. The physician will help shape an emerging care model focused on integrated behavioral and physical healthcare for geriatric patients.
