Prepare and submit eligibility screening and enrollment information for patients, register and navigate new patients, assist with public and private benefit applications, maintain EHR documentation, track enrollment metrics, perform outreach and follow-up, ensure compliance with Medi-Cal/HRSA requirements, and support front office operations.

Provide case management for primarily minor clients: assess needs, develop and implement treatment plans, coordinate housing, entitlements, healthcare and community resources, perform home visits, crisis intervention, maintain documentation, and participate in supervision and team meetings.

Support grant development, contract execution, and strategic partnerships. Track funding opportunities, coordinate proposals and budgets, manage contracts and subcontractor agreements, administer systems (Salesforce, Asana), assist with reports and data entry, support initiative design and events, and represent the agency with partners to increase funding and community engagement.

Provide trauma-informed peer support and crisis diversion services with law enforcement and community partners; perform outreach, assessments, case planning, referrals, documentation (Salesforce), transport clients, and collaborate with clinical staff to connect clients to mental health and substance use resources.

Provide 24-hour peer support at a respite home: outreach, one-on-one coaching, life-skills groups, accompany residents to appointments, assist with housing/food tasks, document progress, and link clients to community resources while maintaining confidentiality and participating in team case management.
