Develops and coordinates complex discharge plans for high-risk members transitioning from acute care. Conducts needs assessments, coordinates with providers and community agencies, documents plans, educates members and caregivers, supports prior authorizations and concurrent reviews, and promotes safe, timely, clinically appropriate transitions between care settings.
