Provides outreach, assessment, care planning and enhanced care management for patients with complex medical and social needs. Coordinates referrals, benefits/housing assistance, documents services in EHR/HMIS, conducts home and community visits, participates in team meetings, and delivers billable case management services while advocating for patients.

Provide friendly front-desk operations for a community health center: reception, registration, scheduling, eligibility screening (prepared two days prior), phone triage, copay collection, data entry, referrals assistance, and front-desk administration.

Provide outreach, engagement, psychosocial assessment, care planning, enhanced care management, referrals, and coordination for patients with complex health and social needs. Conduct home/community visits, document in EHR/HMIS, assist with benefits and housing, support crisis intervention, and participate in interdisciplinary team meetings.

Provide remote and on-site desktop, software, and network support for business, medical, and dental users. Troubleshoot hardware, software, mobile devices, printers and network issues; configure and upgrade PCs and mobile devices; create and manage tickets per SLAs; update documentation and software inventory; participate in PC refreshes, on-call rotation, vendor coordination, and travel to other locations as needed.
