Provide on-site Physical Medicine & Rehabilitation services as a PA in skilled nursing or post-acute rehab facilities. Collaborate with interdisciplinary teams (PT/OT/SLP, PCPs) to assess, manage, and track rehabilitative progress, address barriers to therapy, attend multidisciplinary meetings, and support facility clinical leadership and education initiatives.

Provide PM&R/physiatry consultative care at a designated skilled nursing facility, collaborate with interdisciplinary therapy and PCP teams, track therapy progress, manage rehab-related medical issues, lead clinical initiatives, and participate in facility education and multidisciplinary meetings.
