Provide restorative nursing care to residents per individualized care plans: assist with mobility exercises, transfers, ADLs, catheter support, skin integrity, documentation, and coordinate with Rehabilitation and Nursing to promote independence and monitor condition changes.

Assist with meal preparation, tray assembly, and timely meal delivery; follow diet orders and portion control; maintain kitchen and dining sanitation; wash dishes, stock supplies, and support resident care.

Provide licensed practical nursing care in a skilled nursing facility, including medication administration, treatments, and possible supervision of nurse assistants. Work rotating shifts (day/night, part-time, PRN) to support resident care and collaborate with nursing and therapy staff to ensure high-quality post-acute care.

Prepare meal trays, wash dishes, and assist with dietary department tasks in a skilled nursing/post-acute facility. Support residents by following dietary instructions, maintaining cleanliness, and performing other assigned food service duties.

Greet and assist visitors, handle front desk receptionist duties, and support accounts payable tasks as needed. Full-time shifts Monday–Friday or Tues–Thurs with benefits for full-time staff.

Provide companionship and non-medical support to residents in a skilled nursing facility. Duties include sitting with residents, tidying rooms, answering call lights and communicating needs to nursing staff, delivering meals, passing out snacks and water, and assisting with wheelchair transport to activities and dining. No prior healthcare experience required; training provided.

Create and maintain daily, bi-weekly, and monthly nursing schedules for RNs, LVNs, and CNAs. Fill open shifts, manage call-outs and vacations, monitor labor hours to minimize overtime and meet state staffing mandates, maintain attendance and shift logs in scheduling software, and coordinate staffing needs with the Director of Nursing.

Provide regional clinical leadership across skilled nursing facilities, ensuring regulatory compliance, infection prevention, MDS oversight, survey readiness, and quality improvement. Support and train Directors of Nursing and MDS teams, lead audits and 5-Star initiatives, guide plans of correction, evaluate staffing and workflows, and serve as interim DON as needed. Frequent regional travel required.

Provide regional support and training to MDS coordinators and clinical staff in nursing homes. Conduct meetings, develop clinical tracking for CMS 5-star ratings, assist with RAC audits and regulatory compliance, oversee MDS department, perform facility visits, create action plans, support clinical risk management, and assist defense attorneys with litigation-related documentation as needed.

Oversee facility resident assessment processes (MDS/PDPM), ensure timely accurate assessments, lead PDPM meetings, create clinical reporting, track QM/QASP, review MDS/PDPM claims, provide training and regional workshops, and coordinate interdisciplinary MDS completion for new admissions.

Training role assisting a licensed administrator in managing long-term care facility operations: analyze operations, liaison with residents/families/community, support budgeting, compliance, HR, vendor contracts, QA, safety, collections, and employee engagement under direct preceptor supervision.
