Assists the Director of Nursing with nursing department management, resident care oversight, regulatory compliance, staff recruitment and development, performance management, clinical programs, and quality improvement. Provides direct resident care as needed, conducts rounds, coordinates with interdisciplinary teams, leads meetings, prepares reports, and facilitates ongoing nursing education while ensuring resident safety and regulatory compliance.

Provides welcoming reception services in a post-acute healthcare facility. Greets residents, families, visitors, and staff; answers phones; manages sign-in procedures; responds to questions; maintains reception areas; handles mail, records, logs, messages, and basic clerical tasks; and protects confidentiality while delivering excellent customer service.

Provides social, emotional, and psychosocial services to skilled nursing residents and families. Responsibilities include assessments, individualized care plans, counseling, healthcare decision support, discharge planning, community referrals, appointment coordination, documentation, interdisciplinary collaboration, and staff education. The role requires advocating for resident well-being, maintaining accurate records, and supporting residents through illness, disability, grief, and life transitions.

Coordinates nursing department staffing, schedules, attendance records, employee documentation, interviews, licensing verification, orientation, and staffing communications. Maintains coverage based on resident needs, census, and budget; prepares reports and supports nursing leadership, committees, compliance, confidentiality, safety, and facility operations.

Provides occupational therapy treatments under a licensed Occupational Therapist in a skilled nursing and post-acute setting. Responsibilities include direct resident care, implementing individualized treatment plans, clinical documentation, maintaining safety and infection-control standards, participating in care conferences, training Restorative Aides, and communicating resident progress, treatment goals, physician updates, and discharge planning.

Oversees long-term care dietary operations, including meal planning, food preparation, service, resident dietary needs, food safety, regulatory compliance, budgeting, purchasing, staffing, scheduling, and training. Coordinates with nursing and other departments, maintains diet orders and nutritional records, conducts resident assessments, supports quality assurance activities, and ensures a clean, safe, sanitary kitchen environment.

Performs food service and kitchen support duties, including setting up meal trays, serving meals, washing dishes, operating and monitoring the dishwasher, preparing snacks, storing groceries, cleaning kitchen surfaces and floors, and removing trash. Must maintain sanitation standards, obtain a food handler certificate, follow recipes and procedures, and work cooperatively with dietary staff. The role requires frequent standing, walking, pushing, pulling, and lifting up to 50 pounds.

Provides overnight clinical care to post-acute residents, including assessments, medication and IV therapy administration, monitoring, documentation, and coordination with physicians. Supervises LVNs and CNAs, responds to changes in resident condition, and maintains accurate shift records while working a fixed Thursday-through-Sunday NOC schedule.

Provides licensed practical nursing care in a skilled nursing facility, including medication administration, treatments, resident support, and possible supervision of nurse assistants. The role offers full-time overnight or weekend schedules and PRN day or night shifts. Candidates must hold a current, unencumbered Colorado LPN license; post-acute or skilled nursing experience is preferred.

Provide licensed practical nursing care in a skilled nursing facility, including medication administration, treatments, resident care, and possible supervision of nursing assistants. Leads the rehabilitation department, supervising PT, OT, SLP, assistants, aides, and students. Responsibilities include coordinating staffing, providing direct patient care, monitoring therapy quality, developing clinical programs, ensuring regulatory and documentation compliance, managing productivity and billing requirements, mentoring staff, supporting onboarding and education, and collaborating with facility leadership, residents, families, physicians, and referral partners.

Provides administrative support for a skilled nursing facility’s business office, including recordkeeping, data entry, filing, scanning, paperwork processing, phone support, billing and insurance assistance, and communication with residents, families, staff, and vendors. Maintains confidentiality and supports the Business Office Manager and facility leadership while handling multiple priorities accurately and professionally.

Training role supporting the administration of a long-term care facility under a licensed preceptor. Responsibilities include analyzing operations, assisting with budgeting, compliance, staffing, HR, risk management, vendor contracts, clinical quality, accounts receivable, employee engagement, community relations, and supervision of facility employees.
