Provide relationship-based case management for high-need Medicaid/Medicare members: outreach via phone/SMS, create shared care plans, coordinate appointments and referrals, support clinical team with delegated tasks, meet encounter metrics, and perform community site visits 2-3 times monthly.

Provide bilingual (English/Spanish) case management for high-need Medicaid/Medicare members: outreach, engagement, health action and shared care plan support, coordinate referrals and appointments, collaborate with interdisciplinary care team, and meet encounter metrics while performing most duties remotely with periodic in-person community visits.

Lead FP&A work including budgeting, forecasting, and monthly reporting. Build and maintain financial models, support quarterly forecasts and investor reporting, manage commissions, analyze vendor and AR/claims, and surface financial insights to leadership to improve cash flow and forecast accuracy.

Coordinate and schedule patient referrals and appointments, verify insurance eligibility, manage ROIs and authorized representatives, document activities in internal systems, and collaborate with care teams to ensure timely, HIPAA-compliant care coordination and continuous process improvement.

Partner with Clinical and Operations leaders to strengthen manager effectiveness, org design, performance practices, and people infrastructure. Coach managers on performance and employee relations, lead people programs (performance reviews, talent assessments, succession planning), drive cross-functional projects, use data to guide recommendations, and ensure employment law compliance across a distributed workforce.

Lead patient-focused product marketing: own patient research and segmentation, positioning and messaging, lifecycle and onboarding experiences, go-to-market for product and service launches, and measurement (A/B tests, funnel metrics). Bridge product and patient insight to inform roadmap, partner with growth, brand, clinical and operations, and ensure compliant, trust-preserving patient communications that improve activation, retention, and conversion.

Build and run the compliance function for a fast-growing Medicaid/Medicare-focused digital health provider. Manage regulatory and program compliance (CMS, state Medicaid, value-based contracts), lead HIPAA privacy and security efforts (BAA management, workforce training), respond to audits and regulator requests, partner with product, engineering, clinical, and operations to assess risk, and create scalable compliance processes, policies, training, and risk registers to enable compliant growth.
