Provide relationship-based outreach and case management for high-need Medicaid/Medicare members, conduct telephonic/SMS follow-up, support care planning, coordinate appointments and referrals, collaborate with interdisciplinary team, meet encounter metrics, and perform occasional in-person community visits.

Provide relationship-based case management for high-need Medicaid/Medicare members: outreach via phone/SMS, create shared care plans, coordinate appointments and referrals, support clinical team with delegated tasks, meet encounter metrics, and perform community site visits 2-3 times monthly.

Perform outreach and engagement for high-need Medicaid/Medicare members, maintain caseload using relationship-based and evidence-based approaches, coordinate appointments and referrals, support clinical team with delegated tasks, meet encounter metrics, document in electronic platforms, and connect members to community supports. Role is bilingual (Spanish/English), based in California, and aligned to Pacific Time business hours.
