Teams at STARS

Provide direct client care in a 16-bed inpatient mental health rehabilitation center, administer medications, perform basic medical interventions and education, and support multidisciplinary treatment to increase client independence and functioning.

Provide oversight and care management for clients in the Starlight program, delivering case management/linkage, assessments, screenings, treatment planning and monitoring, and rehabilitation services. Work in community, clinic, home, and occasional telehealth sessions across Santa Clara County; transport clients and perform home-to-home visits. Collaborate with a multidisciplinary team focused on prevention and early intervention for individuals at risk of early psychosis and support families.

Provide intensive case management, outreach, engagement, medication support, counseling, peer groups, and supportive services for transition-age youth (18-25) with serious mental health issues. Deliver trauma-informed, culturally responsive community-based services and coordinate care for clients and families referred to the TAY program.
