Lead and operationalize new market launches for Habitat Health's PACE program, develop repeatable launch playbooks, enable Growth team operations, gather frontline feedback for process improvements, track performance, and coordinate cross-functional and external stakeholders to scale enrollment and outreach.

Supervise Social Work, Participant Advocacy, and Behavioral Health teams within a PACE center. Ensure accurate, compliant, and timely payroll runs; manage tax filings (941, W-2, 1099); handle off-cycle/termination pays and employee inquiries; configure payroll system workflows; document controls; coordinate benefits deductions; support finance/HR projects and provide payroll data for budgeting and forecasting.

Provide remote pre-enrollment psychosocial assessments for prospective PACE participants, using standardized geriatric tools, assess capacity and safety, perform crisis intervention and home-safety evaluations, document in Epic per regulatory standards, collaborate with interdisciplinary teams, and occasionally travel for in-person high-risk evaluations to support safe enrollment and care planning.

Lead ownership and operations for a portfolio of healthcare applications (beginning with TalkDesk). Manage vendor relationships, implementations for new centers, integrations (Epic, Salesforce), training, adoption, documentation, and change management. Drive system performance, roadmap planning, and cross-functional coordination to support rapid PACE expansion and operational readiness.

Build and ship end-to-end AI-enabled tools, agents, copilots, and automations embedded in clinical and operational workflows. Own architecture, data integrations, prompts, evaluation, monitoring, and adoption. Embed with frontline teams to redesign workflows, measure impact, and iterate while ensuring privacy, security, and regulatory compliance.

Lead state-by-state PACE market expansion through regulatory research, RFP monitoring, application and licensing project management, stakeholder and regulatory engagement, and advocacy. Prepare narratives and coordinate cross-functional teams to execute market entry and licensing/renewal processes with independent, action-oriented execution.

Lead and scale outreach and enrollment operations across multiple PACE centers. Manage and develop Outreach and Enrollment supervisors and teams, implement growth systems and workflows, drive enrollment performance using data, align with clinical teams and central growth, and ensure exceptional participant experience while expanding centers.
