Supervise daily operations of a PACE clinic while ensuring participant-centered care for older adults. Manage supplies, vendors, facility and equipment needs, and provide after-hours coverage as needed. Deliver hands-on nursing care including triage, emergency response, medication and vaccine administration, wound care, home care, and patient education.

The Enrollment Registered Nurse leads clinical enrollment activities for a PACE program. Responsibilities include nursing, home safety, and eligibility assessments; Nursing Facility Level of Care documentation; presenting findings to interdisciplinary teams; educating prospective participants and caregivers; maintaining accurate EHR records; supporting regulatory compliance; and contributing to outreach and process improvement. The role requires travel throughout the service area and collaboration across clinical and community settings.

Provide clinical leadership for the PACE center by overseeing nursing and clinic teams, ensuring regulatory compliance (PACE, DHCS, CMS), supervising and developing staff, managing care transitions, driving KPIs and quality initiatives, supporting clinic operations and audit readiness, and delivering direct clinical care when needed.

Supports the Product team by turning ambiguous problems into scalable systems and measurable workflows. Responsibilities include process mapping, automation, research, analysis, documentation, dashboard and reporting development, stakeholder coordination, frontline observation, tool rollout, onboarding, training, adoption measurement, and continuous improvement. The role uses AI and operational data to reduce manual effort, improve throughput and quality, and standardize work across clinical, operational, enrollment, and enterprise teams.

Own product strategy, roadmaps, backlogs, delivery, adoption, and lifecycle management for healthcare technology applications supporting PACE operations. Lead discovery, requirements, vendor relationships, implementations, integrations, go-lives, training, change management, and success measurement across platforms including Talkdesk, Salesforce, and Epic. Partner with clinical, operational, growth, technical, and vendor teams to improve workflows and scale technology across new healthcare centers.

Leads onboarding, training, practicum, shadowing, competency validation, and skills signoffs for PT, OT, Activities Coordinator, and PCA teams across PACE centers. Delivers educational programs, oversees preceptor checklists, supports new center launches, and serves as an operational subject matter expert. Partners with leadership and central learning teams to improve workflows, SOPs, playbooks, training resources, compliance, consistency, and team capability across assigned California centers.

Build and ship end-to-end AI-enabled tools, agents, copilots, and automations embedded in clinical and operational workflows. Own architecture, data integrations, prompts, evaluation, monitoring, and adoption. Embed with frontline teams to redesign workflows, measure impact, and iterate while ensuring privacy, security, and regulatory compliance.

Own the roadmap and delivery of Habitat Health's data products and infrastructure. Serve as the bridge between Product and Data, perform discovery, write technical specs, run sprints, do hands-on SQL/Git validation, design and ship dashboards, triage ad-hoc requests, and use AI tools to accelerate work across platform, integrations, and reporting.
