Lead and operationalize new market launches for Habitat Health's PACE program, develop repeatable launch playbooks, enable Growth team operations, gather frontline feedback for process improvements, track performance, and coordinate cross-functional and external stakeholders to scale enrollment and outreach.

Supervise Social Work, Participant Advocacy, and Behavioral Health teams within a PACE center. Provide coaching, oversee enrollment and advocacy, manage complex participant issues, coordinate with interdisciplinary teams, build community partnerships, ensure regulatory and program alignment, drive quality improvement, and support participant-centered care and retention.

Provide clinical leadership for the PACE center by overseeing nursing and clinic teams, ensuring regulatory compliance (PACE, DHCS, CMS), supervising and developing staff, managing care transitions, driving KPIs and quality initiatives, supporting clinic operations and audit readiness, and delivering direct clinical care when needed.
