The Director of Partnerships will build and manage relationships with healthcare partners, improve partner-facing workflows, resolve operational issues, and create scalable partnership structures supporting new center launches and market expansion. The role requires cross-functional leadership, executive communication, strategic problem-solving, and operational execution in a growing healthcare startup. The position is remote for California-based candidates and requires travel up to 25%.

Supervise daily operations of a PACE clinic while ensuring participant-centered care for older adults. Lead and support nurses and medical assistants, assign duties, manage schedules, oversee EMR documentation, provide coaching and training, and support quality and compliance initiatives. Manage supplies, vendors, facility and equipment needs, and provide after-hours coverage as needed. Deliver hands-on nursing care including triage, emergency response, medication and vaccine administration, wound care, home care, and patient education.

The Enrollment Registered Nurse leads clinical enrollment activities for a PACE program. Responsibilities include nursing, home safety, and eligibility assessments; Nursing Facility Level of Care documentation; presenting findings to interdisciplinary teams; educating prospective participants and caregivers; maintaining accurate EHR records; supporting regulatory compliance; and contributing to outreach and process improvement. The role requires travel throughout the service area and collaboration across clinical and community settings.
