About the Role

Location: Remote - Canada based only

Title: Care Coordinator

We’re hiring a full-time remote Care Coordinator to be the first voice a patient – or their family -hears when reaching out to Sailor Health. In this role, you’ll guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

This role is a unique blend of patient support, care coordination, and emotional guidance. You’ll be the steady hand helping each new patient navigate the journey into care, from matching with the right therapist to feeling confident and understood every step of the way. It’s a perfect fit for someone who thrives at the intersection of empathy and execution, and who finds meaning in turning a moment of vulnerability into one of reassurance and trust.

This role is especially well-suited for someone with an interest in mental health and healthcare access – since you’ll often be the first emotional point of contact for individuals seeking care.

Who You Are
  • Empathetic and clear communicator. You speak with kindness, listen actively, and make others feel at ease – even during difficult or emotional conversations.

  • Highly organized and detail-oriented. You can juggle multiple patients at different stages in the process and never let things slip through the cracks.

  • Tech-comfortable. You’re comfortable learning tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and pick up new software quickly.

  • Mission-aligned. You care deeply about improving care for older adults and are excited to play a key role in someone’s first step toward better mental health.

  • Experience welcome, not required. You may have worked in healthcare, customer service, or an administrative support role before – but most importantly, you’re organized, compassionate, and eager to grow.
