Job Summary


 The Case Manager II – LVN provides advanced care coordination, member outreach, and monitoring support for a defined caseload under the supervision of a Registered Nurse (RN). Building upon foundational case management responsibilities, this role contributes to care plan execution, quality improvement initiatives, and interdisciplinary collaboration. The Case Manager II plays a key role in supporting program effectiveness, ensuring compliance with documentation standards, and identifying opportunities to improve member outcomes and operational workflows.

Functions & Job Responsibilities


All responsibilities of Case Manager I, plus:

  • Conduct routine follow-ups and symptom monitoring.
  • Support interdisciplinary case conferences by preparing case summaries and contributing relevant updates
  • Maintain and update care plan documentation in alignment with RN direction and established guidelines
  • Assist with data collection for trends impacting quality, utilization, and member outcomes.
  • Participates in quality improvement and performance initiatives.
  • Support the development, refinement, and standardization of care management workflows and processes
  • Monitors adherence to assigned program timelines and documentation standards.
  • Identify operational improvement opportunities and escalate system or workflow gaps to leadership
  • Serve as a resource to Case Manager I staff as appropriate
  • Additional duties, as assigned.

Licensures and Certifications

  • Active, unrestricted California LVN license.
  • 3–5 years of case management or managed care experience.
  • Demonstrated experience in care coordination, chronic disease management, or population health programs
  • Critical thinking and problem-solving skills.

  • Ability to manage time effectively, understand directions, and work independently in a fast-paced environment
  • Demonstrated flexibility, organization, and self-motivation
  • Highly adaptable to change
  • Bilingual in Mandarin/Cantonese, Vietnamese, Spanish, or Korean, preferred

 

Wage Range: $32.00/hour - $45.00/hour


Physical & Working Environment.

 

Physical requirements needed to perform the essential functions of the job, with or without reasonable accommodation:

  • Must be able to travel when needed or required  
  • Ability to operate a keyboard, mouse, phone and perform repetitive motion (keyboard); writing (note-taking)
  • Ability to sit for long periods; stand, sit, reach, bend, lift up to fifteen (15) lbs.

 

Ability to express or exchange ideas to impart information to the public and to convey detailed instructions to staff accurately and quickly.

 

Work is performed in an office environment and/or remotely. The job involves frequent contact with staff and public. May occasionally be required to work irregular hours based on the needs of the business.

Salary ranges posted on the job posting are based on California wages. Salary may be higher or lower depending on the candidate’s state residency.
