Description

Vineyard Hills Health Center, 99-bed skilled nursing facility, is now hiring for Social Services Director! We are looking for someone who has compassion and wants to help make a difference in the lives of our residents. This is a full-time position with 5 days a week.


Social Services Director – Essential Duties and Responsibilities

  • Conduct interviews with new residents, their families, and/or responsible parties to assess psychosocial needs, document findings, and develop individualized plans of care.
  • Complete and update resident psychosocial assessments and care plans at least quarterly and as needed to reflect changes in the resident's condition or circumstances.
  • Serve as a resident advocate by communicating with family members, representatives, healthcare providers, community agencies, and other appropriate resources to coordinate referrals and secure services that support residents' physical, emotional, mental, and psychosocial well-being in accordance with federal and state regulations.
  • Collaborate with residents, their representatives, and the interdisciplinary care team to develop and coordinate safe and appropriate discharge plans.
  • Facilitate resident support groups and/or Resident Council meetings in accordance with facility policies and procedures, as assigned.
  • Assist in the resolution of resident and family concerns, requests, and grievances in collaboration with the Administrator and other appropriate staff.
  • Provide information and assistance to residents in completing applications for state assistance programs and accessing available community resources.
  • Keep the immediate supervisor informed of significant issues, concerns, or unusual situations, taking appropriate corrective action when necessary or recommending effective solutions.
  • Perform all duties in a timely, accurate, and professional manner while adhering to company policies, procedures, and regulatory requirements.
  • Foster positive working relationships with residents, families, physicians, community agencies, and all facility staff to promote teamwork, quality resident care, and a positive work environment.
  • Report resident health concerns promptly to the nurse on duty and actively participate in Interdisciplinary Team (IDT) meetings to communicate and follow up on resident needs and care concerns.
  • Maintain strict confidentiality of resident information in compliance with HIPAA and all applicable privacy regulations.
  • Perform other related duties and responsibilities as assigned or required to support the overall operation of the facility and the Social Services Department.

Salary: $26.00-$32.00 DOE

Requirements

Requirements

1. Preferred Degree in Social Work or in any related field.

2. Previous training and experience in skilled nursing administration is preferred.

3. Has good verbal and written communication skills and demonstrates capability to work effectively with all employees.

4. Demonstrates interest and ability to work effectively with geriatric and disabled persons as well as their family members and/or  responsible parties.

5. Has the ability to generate enthusiasm and cooperation from others and to develop and maintain good interpersonal relationships with physicians, discharge planners, social workers and other community contacts.

6. Has the ability to manage multiple tasks with frequent interruptions.

7. Has proficient computer skills.

8. Bilingual is preferred.
