Lead and scale the iOS mobile platform end-to-end: own architecture, roadmap, delivery, and reliability. Drive major performance and crash-rate improvements, build observability and automated testing, coordinate closely with Product/Design/Backend, and improve EHR integrations and technical debt while establishing mobile engineering processes and execution cadence.

About Us
At Sully.ai, We’re Building the Most Impactful Healthcare Company on Earth

We believe that access to a great doctor is a basic human right. Today, that’s not a reality. Delays, misdiagnoses, administrative chaos, and burnout plague the system.

Our Mission
One Human, One Doctor.
We enable our customers to staff 30% of their workforce with AI by creating a shared agent architecture for scale and efficiency. All powered by our own patented, world-class models and deployed in real-world care.

About the Role
Sully automates clinical documentation at the point of care — real-time ambient listening, structured medical notes, and agentic AI workflows that live inside the EHR. We're live in 270+ clinics, have processed 2M+ real physician-patient encounters, and run on custom medical models that outperform GPT, Claude an Gemini on clinical benchmarks. We're not a wrapper company.
We're hiring a hands on Head of Engineering, iOS to own and scale our mobile platform. Our iOS app is how thousands of clinicians interact with AI in patient visits, driving Scribe (AI note generation), Receptionist (front desk automation), and a growing suite of in-app clinical agents.
You will be the DRI for iOS engineering: architecture, quality, reliability, and execution. Small team, large surface area, high ownership. If you want to own the iOS platform of a company defining how AI works in healthcare, this is it
───
What You'll Own
End-to-end iOS engineering: execution, architecture through delivery, crash-free rates, EHR sync reliability, and release cadence
Drive-function performance improvements: targeting 50%+ reductions in app launch time, memory usage, and crash rates (95% → 99.5% crash-free rate)
• Observability, alerting, and automated testing systems that give you real signal at scale
Mobile EHR integration excellence: Epic Haiku, Epic HyperSpace, and other mobile platforms
• Drive OKRs, iOS standups, backlog ownership in Linear, and cross-functional coordination with Product, Design, and Backend
Engineering bar across the mobile stack: code quality, PR reviews, release management, operational rigor
───
What We're Looking For
You've built and shipped iOS at scale: 5+ years with Swift/SwiftUI, UIKit, and modern iOS architectures. Consumer-grade apps shipped to the App Store. Not just feature delivery - memory management, race conditions, SDK integration tradeoffs, and CI architecture.
You still write code: Player-coach, not meetings-coach. We evaluate hands-on iOS depth directly in our process.
You've led mobile teams: 3+ engineers, owned release management, QA workflows, and cross-platform coordination.
You've operated in high-intensity environments: YC-backed, early-stage, or zero-to-one. Speed and judgment over process.
Healthcare or high-reliability software experience is a strong signal. You understand what it means when your code is used by clinicians in patient visits.
Ambiguity is your operating system: Incomplete direction → clear technical plan → shipped product. No hand-holding needed.
───
First 90 Days
Take full ownership of iOS engineering: establish yourself as the DRI for all architecture, quality, and delivery decisions
Ensure end-to-end observability and alerting for core iOS workflows: note generation, medical assistant features, and EHR sync
Define: key performance, reliability, and quality metrics (crash-free rate, memory usage, app launch time, user satisfaction)
Identify and ship at least one high-impact improvement: performance optimization, memory leak fix, or critical UX enhancement
Establish execution cadence: OKRs, standups, iOS release process, QA workflows, and automated testing infrastructure
───
Why Sully

Real Product Market Fit: 270+ clinics live, 2M+ encounters processed, physicians choosing to use it every day
Custom medical models: we build our own and they win on clinical benchmarks
You're replacing a genuinely painful workflow (manual documentation = physician burnout), not adding an AI feature
The technical challenges are real and immediate: EHR sync reliability, in-app AI agents, real-time voice, stability at scale
• Series A with real revenue and real customers, scaling fast
