Title:         Information and Assistance Specialist 

Department:     Social Services

FLSA Status:    Non-Exempt

Reports To:        Social Services Program Manager 

 Summary:     Assists in the navigation of the home and community-based long-term care system; provides assessment, information, referral and assistance to older adults, adults with disabilities, and caregivers; performs follow-up to ensure delivery of services; collects, updates and shares information on community resources. 


Essential Functions:

  • Conducts basic needs assessments of older adults and disabled adults to provide them with information and/or referrals to agencies which offer services that match their needs.
  • Assists consumers in accessing needed services and advocates on their behalf when the requests do not require short-term casework level of assistance (e.g. contacting agencies on behalf of clients, filling out forms, writing letters, or making phone calls to set up or confirm appointments).
  • Provides follow-up with consumers and/or agencies to ensure that needed services were received by consumers.
  • Documents services according to program protocol and submits monthly service logs and reports.
  • Develops and implements outreach strategies in collaboration with the Citywide I&A Supervisor.
  • Conduct regular outreach and presentations in Filipino community settings (e.g., churches, community centers, cultural events) to raise awareness of available resources.
  • Establishes contacts and maintains good working relationships with key personnel in other agencies, including community-based service providers in the district and citywide providers.
  • Participates in meetings and professional development training sessions as assigned.
  • Performs other duties as assigned.


Qualifications:

  • Bachelor’s degree related to human services, or high school graduate with two years of related work experience.
  • Experience providing health, social, or aging services, particularly in diverse communities.
  • Familiarity with community-based resources and social service systems.
  • Culturally sensitive and empathetic toward the needs of older adults and adults with disabilities, especially within ethnic and minority populations.
  • Good problem-solving skills and ability to follow through.
  • Excellent verbal and written communication and interpersonal skills.
  • Bilingual and bi-literate preferred. (English, Chinese, Spanish, Vietnamese, etc.)
  • Proficient in MS Office and the Internet.
  • Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
