Who we’re looking for:

The Referral Coordinator plays a vital role in connecting Forge Health’s mission to the community by building partnerships and driving awareness of our behavioral health services across Eastern PA. As a collaborative member of both the clinical and growth teams, this individual develops outreach initiatives, strengthens relationships with providers and community organizations, and helps ensure individuals can easily access the care they need.
This is a field-based business development role focused on building and activating/maintaining referral relationships across Bucks County and surrounding areas. The role requires regular local travel, onsite engagement with referral partners, and active participation in community events. Success depends on flexibility, responsiveness, and the ability to build trust and momentum outside traditional office hours. Incentives are tied directly to referral growth and relationship performance.
Compensation range: $60-95k, this salary range reflects total compensation, which includes base compensation and bonus payouts, however it is not inclusive of benefits and other company perks. Exact compensation may vary based on skills, experience, and location.

What you’ll be doing:

  • Increase referrals for Forge Health facilities within assigned territory.
  • Execute on business development strategies to meet organizational goals, including deepening Forge Health’s referral base which includes organizations such as hospital systems, primary care physicians, detox centers, psychiatric emergency services, and other behavioral health and community providers.
  • Build and maintain strong relationships with key stakeholders, including healthcare providers, community members, government entities, and enrolled or prospective families to ensure satisfaction, retention, and effective advocacy for Forge Health.
  • Participate actively in community events, county meetings, and coalitions related to behavioral health to promote Forge Health services and specialty programs, maintaining a strong presence in the community.
  • Plan and host events, office tours, and educational forums to inform patients, families, community members, and referral sources about Forge Health’s services.
  • Create and implement outreach initiatives to regularly communicate Forge Health’s mission and value to referral sources through various channels, including phone, email, newsletters, written notes, events, and professional networking activities.
  • Collaborate cross-functionally with internal teams (Marketing, Clinical, Intake, Operations) to align outreach strategies, materials, and messaging.
  • Respond to inquiries from community members and other interested parties, conduct follow-ups, and gather feedback to continuously update and follow the RC playbook for outreach strategies.
  • Build and maintain a robust partnership pipeline using CRM platforms and other available systems for documentation.
  • Enhance the organization’s brand image and reputation by increasing representation at outreach events, networking with potential clients and partners to expand the organization’s reach and influence.
  • Identify new business opportunities and collaborate with senior management to align efforts and develop plans for expansion.
  • Ensure compliance with all job responsibilities and adherence to organizational policies and procedures.

What you’ll need:

  • 2+ years prior experience in behavioral health, Healthcare outreach, provider relations, or business development.
  • Working knowledge of behavioral healthcare models, referral processes and provider workflows.
  • Availability and flexibility to work outside standard 9–5 hours as needed to support referral/community partners, which may include travel to ensure cross coverage within the area, as needed.
  • Experience building and maintaining professional relationships with clinicians, practices, or healthcare organizations.
  • Familiarity with CRM systems and comfort managing outreach pipelines and activity tracking.
  • Organized, self-directed and comfortable working independently in a field-based or remote environment, with ongoing engagement with onsite locations.
  • Excellent communication, presentation and relationship-building skills.
  • Valid state license and access to a vehicle.
