POSITION SUMMARY

The RN Clinical Consultant is a key member of Titanium Healthcare’s Enhanced Care Management (ECM) program, providing clinical oversight and guidance to support high-quality, coordinated care for complex member populations. ECM is a benefit under California Advancing and Innovating Medi-Cal (CalAIM), designed to deliver comprehensive, person-centered care that addresses medical, behavioral, and social determinants of health.

In this role, the RN Clinical Consultant partners closely with Lead Care Managers (LCMs) and interdisciplinary team members to ensure care delivery is clinically appropriate, compliant, and aligned with healthcare plan requirements. The Clinical Consultant supports clinical decision-making, risk stratification, and care planning across the ECM population.

The RN Clinical Consultant is responsible for oversight of Comprehensive Assessments, Care Management Plans, Transitions of Care, and clinical escalations, ensuring timely interventions and continuity of care. This role plays a critical part in improving health outcomes, reducing avoidable utilization, and supporting members through complex care journeys.

ECM services are provided to eligible Medi-Cal members enrolled in managed care plans and focus on sustained engagement, care coordination, and connection to community-based resources. Through collaboration with internal teams and external providers, the RN Clinical Consultant helps ensure members receive the right care, at the right time, in the most appropriate setting.

 

WHERE YOU’LL WORK

This position is remote and is based out of California. Standard business hours are Monday through Friday from 8:00 am to 5:00 pm. 

 

WHAT YOU’LL DO

  • Provide clinical oversight and support to ensure high-quality effective care management for members across the ECM program
  • Collaborate closely with Lead Care Managers (LCMs) and ancillary clinical staff (e.g., RNs, LCSWs, RDs) to support member care needs and clinical decision-making
  • Facilitate access to appropriate healthcare services including primary care, behavioral health, and specialty services, as needed
  • Complete clinical oversight of Comprehensive Assessments and Care Management Plans to ensure accuracy, appropriateness, and alignment with healthcare plan guidelines
  • Lead and/or participate in Emergent and Non-Emergent Clinical Case Conferences to address complex member needs and ensure timely interventions
  • Participate in Interdisciplinary Care Team (ICT) meetings to provide clinical input and recommendations to support coordinated and whole-person care
  • Conduct and/or oversee medication reconciliation to ensure safety and accuracy of member medication regimens
  • Support and guide smooth Transitions of Care (TOC) following emergency room visits and hospitalizations, ensuring timely follow-up and continuity of care
  • Engage members, as appropriate, using clinical expertise and motivational interviewing techniques to support health behavior change and care plan adherence
  • Provide health education within scope of practice to promote improved health outcomes
  • Coordinate care through collaboration with internal teams and external providers, ensuring seamless communication and continuity of care
  • Ensure all clinical activities are documented accurately and in accordance with organizational, regulatory, and healthcare plan requirements
  • Consult and collaborate with clinical leadership as needed for complex cases or escalations
  • Participate in case reviews and clinical discussions to support quality, consistency, and continuous improvement
  • Adhere to all departmental and organizational policies, procedures, and compliance requirements
  • Perform other duties and projects as assigned

  • Fluent in English (written and verbal)
  • Ability to communicate clearly in-person, by phone, and electronically
  • Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation
  • Ability to identify problems and use logic and related information to develop and implement solutions
  • Excellent organizational skills and attention to detail
  • Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice

 Technology Use

  • Frequent use of computers, keyboard, and handheld/mobile devices

  • Competent with computers, email, virtual platforms, and Microsoft Office based programs

WHAT YOU’LL NEED

  • Associate’s degree in nursing
  • Minimum 1+ years of experience as an RN working in a healthcare setting, such as a medical clinic or acute care hospital
  • A current and unrestricted CA state RN license
  • Experience using an EHR/EPM
  • Distraction-free home workspace with a secure internet connection

  • Care management experience preferred
  • Bachelor's degree in nursing 
  • Experience using eClinical Works (eCW)
  • Valid BLS certification from the American Heart Association 
  • Valid CPR Certification 
  • Bilingual in Spanish
