Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge.Job Description:
EDUCATION:
- HS Diploma or General Education Diploma (GED)
TYPICAL EXPERIENCE:
- 1-year recent relevant experience.
SKILLS AND KNOWLEDGE:
- Oral and written communication skills.
- Interpersonal and time management skills
- Ability to work effectively in a fast-paced environment with rapidly shifting priorities and competing demands.
- Ability to work independently with a minimum of direction.
- Ability to exercise discretion and prioritize tasks, seeking input as indicated.
- Intermediate PC skills and word processing skills required.

Pay Range is $26.18 to $34.03 / hourThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
