The Complex Care Navigator supports patients and families living with serious illness by helping them navigate the healthcare system and align medical care with the patient's goals, values, and lived experience.
This role serves as a consistent point of contact for patients and caregivers, fostering trusting relationships and helping bridge communication between patients, families, providers, and care teams. The navigator works collaboratively within an interdisciplinary palliative care team to support coordination, communication, and patient-centered care.
Schedule: A 40 hour week, Monday through Friday from 8:00 am to 4:30 pm
Location: 7600 EVERGREEN WAY, Everett, WA 98203
Primary Responsibilities:
  • Enroll patients with serious or complex illness into the palliative care program, explain program services, and serve as a primary point of contact for patients and families.
  • Maybe the ability to conduct work in a hybrid setting, including approximately 70% office-based responsibilities and 30% community-based activities such as home visits and patient outreach.
  • Strong emotional intelligence and ability to navigate sensitive, emotionally complex situations and family dynamics.
  • Strong organizational skills with the ability to manage competing priorities independently.
  • Ability to work independently while remaining highly collaborative with healthcare teams.
  • Maintain confidentiality of patient information in accordance with HIPAA guidelines and Optum policies.
  • Attend required training and maintain compliance with state, federal, OSHA, DOSH, and organizational requirements.
  • Maintain regular and reliable attendance.
  • Performs all other duties as assigned

Required Qualifications:
  • High School Diploma /GED
  • One plus years of computer proficiency experience in Microsoft Office and demonstrates solid computer skills
  • 2+ years of customer service experience
  • Ability to travel 10% of the time within Everett, WA and surrounding communities.
  • Have reliable transportation, with a current and non-restricted state of Washington Driver's License and State-required insurance

Preferred Qualifications:
  • Medical Assistant training or other healthcare-related education
  • Non-licensed and non-certified candidates with relevant experience may be considered
  • Previous experience in healthcare, patient navigation, care coordination, caregiving, social services, or related fields preferred.
  • Experience working with patients with serious illness, complex medical conditions, geriatrics, hospice, or palliative care preferred.
  • Experience supporting diverse patient populations and navigating complex family or social situations preferred.

  • Ability to work independently and as a team, and maintain good judgment and accountability
  • Demonstrated ability to work well with health care providers
  • Strong organizational and time management skills
  • Ability to multi-task and prioritize tasks to meet all deadlines
  • Ability to work well under pressure in a fast-paced environment
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $18 to $32 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
